The following documents are required with your application:

• Birth Certificate
• Current year-to-date report card
• Previous year final report card
• Most recent Standardized Test Scores (for students in grades 2nd-8th)
• Teacher Recommendation. Please provide the name and email address of one current academic teacher (for example, your child’s English, Math, or Science teacher). Once the application is submitted, an email with a link to complete the recommendation will be sent directly to the teacher. Please note that we cannot accept recommendations submitted by parents or from individuals who are not current academic teachers.

• Psychological Educational Evaluation (if applicable)
• Learning Support, Individualized Education Plan or 504 Plan (if applicable)

These documents should be scanned as PDFs and uploaded electronically during the application process. JPEG is not an acceptable file type.

An online application must be completed for every student hoping to be admitted to San Jose Catholic School. There is a $50 application fee, per student. VPK/PK4 students do not pay an Application Fee.

Each prospective student will participate in a New Student Assessment.

• PK3, PK4, and Kindergarten assessments may be conducted individually or in small groups with the respective teachers. The purpose of these assessments is to evaluate each child’s school readiness, ability to communicate with teachers and peers, and current level of knowledge. Each assessment typically takes approximately 30 minutes to complete.
• First and second grade applicants will meet individually with a member of our Learning Support team. During this session, students will be assessed in reading and math and will complete a brief writing sample.
• Third through eighth grade assessments are used to evaluate students’ math and reading skills. This assessment is administered on an iPad and takes approximately 45 minutes to complete. Students are also required to complete a writing sample.

San Jose Catholic School has a rolling admissions process. We will continue to take applications, conduct assessments, and admit students as space is available. Admissions decisions are communicated via email.

The following documents are required for enrollment: 

• Current FL Certificate of Immunization (DH680) 

Florida Statute 232.0332, which became effective on August 1, 1982, requires that prior to admittance to or attendance in a public or non-public school, grades kindergarten through 12, or a preschool, each child present must have on file with the school a certificate of immunization (DH 680) for the prevention of those communicable diseases for which immunizations are required by the Department of Health. 

FL Department of Health regulations require DH 680 Form, Part C, to be signed by hand or by electronic signature in Florida SHOTS by a physician licensed under Chapter 458 MEDICAL PRACTICE or Chapter 459 OSTEOPATHIC MEDICINE, Florida Statutes.

• Current FL School Health Entry Exam (Form 3040)
• VPK Certificate of Eligibility (if applicable)

The Registration Fee is paid at the time of enrollment packet submission.  This fee is paid per family and is non-refundable.
